About the Jones Foundation & Team Schnak Strong Fund
The Jones Foundation has teamed up with Team Schnak Strong Fund to provide a $3,000 Diabetes Expense Grant to eligible applicants in Coffey, Lyon and Osage Counties to help offset expenses associated with the treatment of Childhood Type 1 Diabetes. In addition to this program, you may also apply for the Children’s Medical Expense Grant if needed.
Program Rules
- This grant is payable in the amount of $3,000 per applicant, per year up to the age of 21 and may be reapplied for annually.
- There is no income requirement for this grant.
Eligibility
To be eligible for this program, applicants must:
Be under age 21.
Be a US citizen or non-US citizen with Lawful Immigration Status of Permanent Resident.
Have lived continuously in Coffey, Lyon, or Osage County, Kansas for at least one year immediately before applying.
Required Documents
Along with your application, you’ll need to submit the following documents:
- Proof of Type 1 Diabetes diagnosis by either a copy of signed Physician’s Letter OR printed document/screenshot from online medical portal.
- School records for proof of residency in Coffey, Lyon, or Osage county. If applicant is beyond school age, please submit first page of most recent tax return verifying address.
- Birth Certificate for proof of citizenship and age verification.
