About the Program
The Jones Foundation assists qualified young adults ages 18–20 who have a current Individualized Education Program (IEP) or Person-Centered Plan and are receiving special education (at a local school district or specialized school) or Community Developmental Disability Organization (CDDO) services. Eligible participants are individuals whose plans include clearly defined post-high school transition goals and whose assessments or documented needs indicate a requirement for continued, intensive support in the areas of independent living, integrated employment, and community participation.
This grant is specifically intended for individuals who require structured support, systematic instruction, and guided practice to successfully achieve their post-secondary outcomes, rather than those able to independently access post-secondary education or employment without additional services.
First-time Transitions Grant applicants are eligible for an award of $2,000. The award amount is $1,500 for second-time recipients and $1,000 for third-time recipients.
Program Rules
- Renewal applications may be completed after 1 calendar year from receipt of first award and 1 calendar year from receipt of second award.
- This grant can be awarded up to three times per applicant.
- Funding for second and third-time applicants will be contingent upon demonstrated meaningful progress toward established goals.
Eligibility
To be eligible for this program, applicants must:
Be age 18-20.
Be a US citizen or non-US citizen with Lawful Immigration Status of Permanent Resident.
Have lived continuously in Coffey, Lyon, or Osage County, Kansas for at least one year immediately before applying.
Have a current IEP or Person-Centered Plan and receive special education or CDDO services.
Have a post-high school transition goal.
